Care Coordinator

The overall responsibility of the Care Coordinator is to provide the highest quality of care available to help patients experience therapy above what they are accustomed to. Care Coordinator employees are expected to maintain a patient first focus and strive to create patients for life, ensuring they receive the best quality service from start to finish. Above all, the Care Coordinator is responsible for demonstrating our core values of quality, service, relationship-centered and applying principles of excellence, caring, ethics, respect, and communication.

ROLE #1: Customer Service (60%)


  1. Reception
    1. Welcome and greet all patients
    2. Answering all inquiries in person or on the telephone in a polite, professional manner.
    3. Keep area clean and organized
    4. Contact referring providers for patient scripts
  2. Scheduling
    1. Schedule patients according to their needs and availability
    2. Facilitates patient flow by informing therapist of patient arrivals and delays
    3. Call patients to confirm their appointment
    4. Verify insurance information for billing purposes
  3. Payments
    1. Ensure copays and self-payment amounts are paid at the time of service
    2. Errors in payment need to be reported to the billing office
    3. Properly document patient records when payment is processed

ROLE #2: Patient Records (30%)


  1. Patient Information
    1. Register new referrals and update existing patient files
      1. Demographics
      2. Contact information
  • Insurance Information
  1. Insurance
    1. Properly input insurance policy information
    2. Contact CIVA with insurance policy number and policy holder
  • Verify patient understands their coverage, financial responsibility and signs their benefit letter
  1. Records Request
    1. Route all medical records request to the Billing Office
    2. Patients will need to fill out a waiver to release their records to ensure proper compliance

ROLE #3: Administrative (10%)


  1. Supplies
    1. Place orders for office supplies
    2. Stocking and organizing all supplies

  • High School Diploma required
  • Knowledge of office and clinical procedures
  • Previous receptionist experience or medical office experience is preferred
  • Customer service experience required
  • Proficient in multi-tasking and time management
  • Excellent phone etiquette and ability to manage all kinds of front desk tools
  • *For Downtown position: Bilingual Required
